Step 2: Print and sign the South Pointe Middle School Policies and Protocols Signature Acknowledgement & Accountability form. Return it to South Pointe at Registration. 

 

Step 3: Read the Walnut Valley Unified School District First Day Parent Packet. 

 

Step 4: Print and sign the Walnut Valley Unified School District Signature and Acknowledgment & Accountability form. Return it to South Pointe at Registration. 

 

Step 5: Print and sign the Voluntary Student Accident Insurance form. Return it at South Pointe Registration. Please click here to view the Voluntary Student Accident Insurance Booklet.

 

6th graders and New Students, please complete the Student Health Assessment. 

 

Optional: Families may choose to complete this Student Residency Questionnaire and return it at Registration.
